Insurance/Financial Information

We can file claims for most different insurance plans, as well as being a Delta Premier, Encore, CAPS (Consumers Association for Professional Services), Premier/Humana, Aetna, and Cigna provider. In order for us to process your insurance claims, we are mandated by the State of Minnesota and the Federal Government to have a Health Insurance Portability and Accountability Act form (HIPPA form) completely filled out and signed by the patient or guardian.

We accept cash, check, Visa, Mastercard, Discover and American Express cards. We also offer Care Credit® patient financing. Patients who choose to pay for the entire amount of their bill with cash or check will receive a 5% discount regardless of insurance status. Seniors (over age 65) will receive a 10% discount for this same program.

An e-statement or mailed statement will be sent to you if there is a difference between our estimate and what the insurance company actually paid.

We make every effort as a courtesy to contact your insurance provider and receive a general breakdown of benefits and we will use this to estimate your deductible and co-pay.
